Chửa trứng bán phần là một trong nhóm bệnh lý hiếm gặp của bệnh nguyên bào nuôi thai kỳ (Gestational Trophoblastic Disease - GTD) có thể gây chảy máu dữ dội, đe dọa tính mạng người bệnh đặc biệt là chảy máu sau hút buồng tử cung. Việc điều trị trở nên khó khăn hơn đối với những người bệnh mong muốn giữ tử cung để giữ khả năng sinh sản. Do đó, can thiệp nội mạch là một phương pháp điều trị xâm lấn tối thiểu có thể đáp ứng nhu cầu vừa cầm máu, vừa giữ chức năng sinh sản cho người bệnh. Chúng tôi báo cáo người bệnh nữ 28 tuổi, tiền sử chửa trứng bán phần, nhập viện vì ra máu âm đạo số lượng nhiều. Xét nghiệm cho thấy β-hCG tăng cao (266.145) IU/L. Hình ảnh CLVT phát hiện khối tăng sinh mạch trong tử cung gây vỡ, xuất huyết ổ bụng, được cấp máu từ nhiều nhánh động mạch vùng chậu. người bệnh được can thiệp nút mạch khẩn cấp bằng keo, hạt PVA và Spongel. Sau thủ thuật, chảy máu được kiểm soát hoàn toàn, huyết động ổn định cho thấy nút mạch chọn lọc là phương pháp an toàn và hiệu quả trong xử trí xuất huyết nặng do bệnh nguyên bào nuôi thai kỳ ác tính.

Intra-abdominal hemorrhage due to hypervascular partial hydatidiform mole: A case report and the role of urgent embolization

Abstract
Partial hydatidiform mole is a rare entity within the spectrum of gestational trophoblastic disease (GTD) that can lead to life-threatening, profuse hemorrhage, particularly following uterine curettage. Treatment becomes more challenging in patients who desire uterine preservation to maintain future fertility. Consequently, endovascular intervention has emerged as a minimally invasive treatment option capable of achieving both definitive hemostasis and reproductive preservation. We report a 28-year-old female with a history of partial hydatidiform mole, presenting with massive vaginal bleeding. Laboratory tests revealed a highly elevated β-hCG level (266,145 IU/L). Computed tomography (CT) imaging identified a ruptured intrauterine hypervascular mass causing hemoperitoneum, supplied by multiple pelvic arterial branches. The patient underwent urgent embolization using glue, polyvinyl alcohol (PVA) particles, and Gelfoam (Spongel). Post-procedure, the hemorrhage was completely controlled and hemodynamics remained stable, demonstrating that selective embolization is a safe and effective method for managing severe hemorrhage associated with malignant gestational trophoblastic disease.
