Phẫu thuật tạo hình che phủ khuyết phần mềm dạng lột găng ngón tay cái ở các trường hợp không thể nối lại phần đứt rời luôn là thách thức và cần được tiến hành sớm. Nhóm nghiên cứu thông báo kết quả bước đầu trường hợp đầu tiên tại Việt Nam được chuyển tự do thì đầu vạt phức hợp ngón chân 1-2 kèm vạt mu chân che phủ và tạo hình ngón tay cái trên người bệnh nam 16 tuổi bị cụt chấn thương kèm lột găng ngón cái, khuyết phần mềm kẽ ngón 1- 2 bàn tay phải do pháo nổ 10 ngày trước đó. Phẫu thuật đã tạo hình lại thành công ngón cái có hình dạng và kích thước phù hợp, che phủ được phần da lột găng xung quanh đốt 1 và ô mô cái, tạo hình lại kẽ ngón 1-2 với độ rộng phù hợp trong một thì phẫu thuật. Từ kết quả của ca lâm sàng và tổng kết y văn cho thấy, phẫu thuật chuyển ngón chân tự do thì đầu tạo hình ngón cái và che phủ tổn thương dạng lột găng có thể được tiến hành an toàn và hiệu quả tại các cơ sở đủ điều kiện nhân lực và trang thiết bị.

Primary microsurgical complex wrapped toes with dorsalis pedis flap transfer for reconstruction of the wide degloving thumb: Case report

Abstract
Reconstruction of gloving thumb soft tissue defects in cases where the amputated part cannot be replanted is always a challenge and should be performed as soon as possible. The research team reported the first case in Vietnam that underwent primary microsurgery to transfer the complex wrapped flap of toes 1-2 with dorsalis pedis flap covering and reconstruction for a 16-year-old male patient with traumatic amputation and degloving thumb combined with soft tissue defect between toes 1-2 of the right hand due to a firecracker explosion accident in last 10 days. The surgery successfully reconstructed the thumb to the appropriate shape and size, covered the degloving skin around the first phalanx of the thumb as well as the interdigital spaces of fingers 1-2 with the appropriate width in only one surgery. Experience from this case in reviewing the literatures to show that the primary microsurgical complex wrapped toes with dorsalis pedis flap transfer to degloving thumb can be performed safely and effectively in facilities with adequate personnel and equipment.
