Thoái hóa cột sống thắt lưng là tình trạng phổ biến, đặc biệt ở người cao tuổi, thường dẫn đến tình trạng hẹp ống sống, thoát vị đĩa đệm và trượt đốt sống. Phẫu thuật hàn xương liên thân đốt qua lỗ liên hợp (TLIF) là giải pháp hiệu quả cho các trường hợp mất vững và biến dạng nặng. So với PLIF, TLIF có tỷ lệ biến chứng thấp hơn (8,7% so với 17%). Tuy nhiên, TLIF kinh điển vẫn tồn tại các hạn chế: khả năng phục hồi độ ưỡn cột sống hạn chế, nguy cơ trồi lồng xương vào ống sống (đặc biệt tại tầng L5-S1) và lún lồng xương. Một loạt những cải biên dành cho phẫu thuật TLIF đã được đề cập trong y văn thế giới nhằm khắc phục các vấn đề nêu trên, đặc biệt khi được tiến hành ở người bệnh lớn tuổi, loãng xương kèm biến dạng cột sống do thoái hoá. Các cải biên bao gồm: 1. cắt bỏ hoàn toàn hai mấu khớp, một phần mấu gai và nửa bản sống dưới giúp giải ép triệt để và tạo thuận lợi tối đa để phục hồi độ ưỡn; 2. dời vị trí của lồng xương PEEK ra phía trước khoang đĩa đệm và; 3. ghép xương bổ sung phía sau lồng xương nhằm thúc đẩy lành xương.

Narrative review of transforaminal lumbar interbody fusion: From conventional technique to modern modifications

Abstract
Degenerative lumbar disc disease is a common health condition, especially among the elderly, often leading cause to spinal stenosis, disc herniation, and spondylolisthesis that require surgery when conservative treatment fails. Transforaminal Lumbar Interbody Fusion (TLIF) is an effective solution for cases of instability and severe deformity. Compared to PLIF, TLIF has a lower complication rate (8.7% vs. 17%). However, classic TLIF still faces limitations: restricted ability to restore lumbar lordosis, risk of cage retropulsion into the spinal canal (especially at the L5-S1 level), and cage subsidence. Factors such as cage placement in the center or posterior half of the disc space and poor bone quality increase these risks. A series of modifications for the TLIF procedure have been documented in global medical literature to address the aforementioned issues, particularly for elderly patients with osteoporosis and spinal deformities due to degeneration. These modifications include: 1. total bilateral facetectomy, partial spinous process resection, and inferior hemilaminectomy to assist in radical decompression and create optimal conditions for restoring lordosis; 2. moving the PEEK cage position further anterior within the disc space; 3. supplementary bone grafting posterior to the cage to promote bone healing.
