Thông báo lâm sàng: Nang ruột đôi trực tràng hiếm gặp, điểm lại y văn, chẩn đoán và hướng điều trị

Nang ruột đôi (intestinal duplication cyst) là dị tật bẩm sinh hiếm gặp của ống tiêu hoá, được đặc trưng bởi sự hiện diện của một cấu trúc nang có thành chứa cơ trơn và lớp niêm mạc tương tự như ruột. Trong đó vị trí ở trực tràng là rất hiếm gặp, chiếm chưa đến 5% các trường hợp. MRI đóng vai trò quan trọng trong chẩn đoán xác định nang ruột đôi trực tràng và phân biệt tổn thương lành tính với ác tính. Phẫu thuật là phương pháp điều trị triệt để, lựa chọn đường mổ tùy vị trí, kích thước và mức độ xâm lấn của nang. Chúng tôi báo cáo trường hợp người bệnh nữ 72 tuổi, có tiền sử phẫu thuật u nang bì vùng tầng sinh môn, nhập viện vì đau vùng hậu môn. Hình ảnh MRI cho thấy khối nang vùng cùng cụt đè đẩy trực tràng, ranh giới rõ, không xâm lấn. người bệnh được phẫu thuật bóc tách toàn bộ khối u qua đường sau cùng cụt. Giải phẫu bệnh sau mổ là nang ruột đôi trực tràng áp xe hóa, không ác tính. Sau phẫu thuật, người bệnh hồi phục tốt và ra viện sau 8 ngày. Ca bệnh này cho thấy vai trò của chẩn đoán hình ảnh và phẫu thuật triệt để trong điều trị nang ruột đôi trực tràng.

Abstract
Intestinal duplication cyst is a rare congenital malformation of the gastrointestinal tract, characterized by a cystic structure with a smooth muscle wall and mucosal lining similar to that of the intestine. Among them, the rectal location is extremely rare, accounting for less than 5% of cases. MRI plays an important role in confirming the diagnosis of rectal duplication cysts and determining benign from malignant lesions. Surgery is the definitive treatment, with the operative approach depending on the cysts location, size, and degree of invasion. We report a case of a 72-year-old female with a history of perineal dermoid cyst excision, admitted with anal pain. MRI revealed a well-defined presacral cystic mass compressing the rectum without invasion. The patient underwent complete cyst excision via a posterior trans-sacral approach. Histopathological examination confirmed an infected rectal duplication cyst (RDC) without malignancy. The postoperative course was uneventful, and the patient was discharged on postoperative day 8. This case highlights the role of imaging in diagnosis and the importance of complete surgical excision in managing rectal duplication cysts.
