Nhiễm trùng khớp gối nhân tạo là một biến chứng nghiêm trọng sau thay khớp gối toàn phần. Điều trị thường đòi hỏi phối hợp giữa phẫu thuật, lấy bỏ implant, đặt spacer và sử dụng kháng sinh kéo dài. Chúng tôi báo cáo trường hợp một người bệnh nữ 77 tuổi, có tiền sử tăng huyết áp, từng phẫu thuật thay khớp gối trái cách đây 5 năm. Tháng 4/2024, người bệnh phải lấy bỏ khớp nhân tạo do nhiễm trùng, đặt spacer và điều trị kháng sinh tĩnh mạch. người bệnh được phẫu thuật lại thay toàn bộ khớp gối trái bản lề kèm theo phẫu thuật Judet. Hiện tại, người bệnh không có dấu hiệu viêm cấp tính tại và đi lại. Trường hợp này minh hoạ tính phức tạp trong chẩn đoán nhiễm trùng khớp gối nhân tạo ở người bệnh cao tuổi, cũng như những khó khăn trong quyết định điều trị.

Periprosthetic joint infection of the knee following total knee arthroplasty in elderly patient: A case report

Abstract
Periprosthetic joint infection (PJI) of the knee is a serious complication following total knee arthroplasty. Management typically requires a combination of surgical intervention, implant removal, placement of a spacer, and prolonged antimicrobial therapy. We report the case of a 77-year-old woman with a history of hypertension who underwent left total knee arthroplasty five years earlier. In April 2024, she was re-operated for removal of the implant due to infection, placed a spacer, and indicated for intravenous antibiotic treatment. The patient subsequently underwent revision surgery with total knee arthroplasty combined with a Judet procedure. At present, she shows no signs of acute inflammation and has regained walking. This case illustrates the complexity of diagnosing periprosthetic joint infection of the knee in elderly patients, as well as the challenges involved in determining the optimal treatment strategy.
