Điều trị trĩ chảy máu bằng can thiệp nội mạch: Báo cáo chùm ca lâm sàng

Bệnh trĩ là bệnh lý phổ biến có thể điều trị bằng nhiều phương pháp như điều trị bảo tồn, can thiệp nội mạch và phẫu thuật phụ thuộc vào mức độ phức tạp của bệnh và bệnh lý nền của người bệnh. Trong đó, đối các người bệnh có bệnh lý phối hợp như tim mạch, rối loạn đông máu là một trong những nhóm không thích hợp đối với phẫu thuật. Bài báo trình bày 4 ca lâm sàng được điều trị trĩ nội độ II-III chảy máu bằng nút chọn lọc động mạch trực tràng trên dưới hướng dẫn của DSA. Ngay sau can thiệp, các ca đều có cải thiện rõ rệt triệu chứng lâm sàng, không còn chảy máu, giảm kích thước búi trĩ và không ghi nhận biến chứng nặng. Từ đó cho thấy, can thiệp nội mạch là phương pháp điều trị hiệu quả và an toàn cho người bệnh trĩ có bệnh lý nền nặng, rối loạn đông máu, và cả nhóm không có bệnh lý nền, từ đó cung cấp thêm lựa chọn điều trị cho những người bệnh không phù hợp trong điều trị phẫu thuật.

Endovascular intervention for bleeding hemorrhoids: A case series

Abstract
Hemorrhoids is a common condition that can be managed through various approaches, including conservative treatment, endovascular intervention, and surgery, depending on the severity of the disease and the patient’s health condition. Specifically, patients with comorbidities such as cardiovascular disease or coagulation disorders are often not suitable for surgery. This article presents four clinical cases complicated bleeding grade II-III internal hemorrhoids treated with selective superior rectal artery embolization under DSA (Digital Subtraction Angiography) guidance. Immediately after the intervention, all cases showed significant improvement in clinical manifestations, with no further bleeding, reduction hemorrhoid size, and no serious complications were recorded. These results demonstrate that endovascular intervention is an effective and safe treatment for hemorrhoid patients with severe underlying diseases or coagulation disorders, as well as for those without comorbidities. This provides an additional therapeutic option for patients who are not eligible for surgical treatment.
