Kết quả tái tạo dây chằng chéo trước khớp gối bằng kỹ thuật tất cả bên trong tại Bệnh viện Đà Nẵng

Đặt vấn đề: Kỹ thuật tái tạo dây chằng chéo trước tất cả bên trong bằng gân cơ bán gân chập bốn là kỹ thuật mới với nhiều ưu điểm. Nghiên cứu nhằm đánh giá kết quả phẫu thuật của kỹ thuật này và nhận xét các yếu tố ảnh hưởng kết quả điều trị.
Đối tượng và phương pháp nghiên cứu: Nghiên cứu tiến cứu trên 45 người bệnh được phẫu thuật tái tạo dây chằng chéo trước tất cả bên trong tại Bệnh viện Đà Nẵng từ tháng 04/2022 đến tháng 03/2023.
Kết quả: Đường kính mảnh ghép trung bình là 8,45 ± 0,54mm (7 - 10mm). Chiều dài mảnh ghép trung bình là 61,7 ± 2,7mm (58 - 70mm). Sau mổ 6 tháng, có 40 trường hợp gối vững, dấu hiệu ngăn kéo trước, dấu hiệu Lachman và Pivot Shift đều âm tính. Điểm Lysholm cao hơn có ý nghĩa thống kê so với trước mổ (93,3 ± 4,72 so với 59,20 ± 5,42, p < 0,001).
Kết luận: Tái tạo dây chằng chéo trước bằng kỹ thuật tất cả bên trong cho kết quả tốt về độ vững và chức năng khớp gối. Yếu tố kỹ thuật quan trọng là chuẩn bị mảnh ghép gân đường kính lớn, chiều dài phù hợp.

Result of anterior cruciate ligament reconstruction using the all – inside technique at Da Nang Hospital

Abstract
Introduction: All-inside anterior cruciate ligament reconstruction technique using quadrupled semitendinosus tendon is a new method with advantages. This study aims to evaluate the surgical outcomes of this technique and to identify factors affecting the results.
Patients and Methods: A prospective study was conducted on 45 patients who underwent all-inside anterior cruciate ligament reconstruction at Da Nang Hospital from April 2022 to March 2023.
Results: The mean graft diameter was 8,45 ± 0,54mm (7 - 10mm). The mean graft length was 61,7 ± 2,7mm (58 - 70mm). Six months postoperatively, 40 cases had stable knees, with negative anterior drawer, Lachman and Pivot Shift tests. The Lysholm score was significantly higher than preoperatively C, p < 0,001).
Conclusions: All-inside anterior cruciate ligament reconstruction provides good outcomes in terms of stability and function of knee. An important affecting technical factor is to prepare a graft with appropriate diameter and length.
