Đánh giá kết quả phẫu thuật nội soi điều trị ung thư dạ dày tại Bệnh viện Hữu nghị Việt Đức

Đặt vấn đề: Phẫu thuật nội soi (PTNS) điều trị ung thư dạ dày đã được chứng minh có nhiều ưu điểm hơn so với phẫu thuật mở. Tuy nhiên việc đánh giá kết quả sau phẫu thuật nội soi điều trị ung thư dạ dày đặc biệt giai đoạn bệnh tiến triển vẫn cần nghiên cứu thêm. Nghiên cứu này nhằm đánh giá tính an toàn và hiệu quả của phẫu thuật nội soi trong điều trị ung thư dạ dày.
Đối tượng và phương pháp nghiên cứu: Nghiên cứu hồi cứu 100 người bệnh UTDD được PTNS cắt dạ dày từ tháng 1/2015 đến tháng 1/2020 tại Bệnh viện Hữu nghị Việt Đức. Đánh giá kết quả sớm và kết quả xa sau phẫu thuật cắt dạ dày nội soi.
Kết quả: Có 93 người bệnh được cắt bán phần dạ dày và 07 người bệnh cắt toàn bộ dạ dày. Thời gian mổ trung bình là 209,46 ± 65,14 phút, lượng máu mất trung bình là 175,88 ± 56,04 ml. Số hạch nạo vét được trung bình là 16,24 ± 5,58 hạch. 45 người bệnh có di căn hạch. Thời gian sống sau mổ 1 năm là 84% và sau 5 năm là 71%. 1 trường hợp tai biến vết thương bên tĩnh mạch cửa trong mổ do quá trình nạo vét hạch, phẫu thuật viên đã tiến hành chuyển mổ mở, diễn biến người bệnh sau mổ có suy gan cấp và tử vong sau mổ. 3 trường hợp viêm phổi và 1 trường hợp nhiễm trùng vết mổ điều trị nội khoa, 2 trường hợp hẹp miệng nối điều trị nong miệng nối.
Kết luận: Phẫu thuật nội soi điều trị ung thư dạ dày là an toàn và đáp ứng yêu cầu mặt ung thư học.

Evaluating the results of laparoscopic surgery for gastric cancer at Viet Duc University Hospital

Abstract
Introduction: Laparoscopic gastrectomy has been widely adopted for treatment of gastric cancer and demonstrated several advantages compared with conventional surgery. However, further research is needed to evaluate the outcomes of laparoscopic surgery for gastric cancer, especially in advanced stages. This study aims to assess the safety and effectiveness of laparoscopic surgery in the treatment of gastric cancer.
Patients and Methods: We conducted a retrospective study on 100 patients with gastric cancers who underwent laparoscopic gastrectomy at Viet Duc University Hospital between January 2015 and January 2020. Short-term surgical outcomes and long-term survival were recorded and analyzed.
Results: Subtotal gastrectomy was performed in 93 patients and total gastrectomy in 7 patients. The mean operative time was 209.46 ± 65.14 minutes, and the mean intraoperative blood loss was 175.88 ± 56.04 ml. The mean number of retrieved lymph nodes was 16.24 ± 5.58. 45 patients had lymph node metastases. The 1-year survival rate was 84% and the 5-year survival rate was 71%. One case involved a portal vein wound during surgery due to lymph node dissection, was converted to open surgery. The patient subsequently developed acute liver failure and died after surgery. Postoperative complications included pneumonia (3%), surgical site infection (1%), and 2 cases of anastomotic stenosis (2%) were treated with anastomotic dilation.
Conclusions: Laparoscopic gastrectomy for gastric cancer is safe and oncologically feasible, with acceptable postoperative morbidity and favorable long-term survival outcomes.
