Schwannoma phần lớn là khối u lành tính phát triển chậm, có nguồn gốc từ tế bào schwann, thường gặp ở chi thể, đầu cổ, trung thất, sau phúc mạc và khung chậu, trong khi schwannoma gan lại vô cùng hiếm gặp. Chúng tôi trình bày ca bệnh nữ 53 tuổi vào viện vì đau bụng thượng vị, qua thăm khám được chẩn đoán trước mổ là u gan phải theo dõi sarcoma. người bệnh được phẫu thuật cắt gan phải kèm cắt đường mật ngoài gan và tái lập lưu thông mật - ruột kiểu Roux-en-Y. Kết quả giải phẫu bệnh cho thấy các tế bào hình thoi nhân bầu dục, sắp xếp thành bó, hóa mô miễn dịch S-100 dương tính, phù hợp với schwannoma gan. Sau phẫu thuật người bệnh hồi phục tốt và ra viện. Schwannoma gan là bệnh lý hiếm gặp, triệu chứng lâm sàng và hình ảnh học không đặc hiệu, chẩn đoán xác định dựa vào mô bệnh học và hóa mô miễn dịch. Phẫu thuật triệt để được coi là phương pháp điều trị tối ưu vì vừa loại bỏ khối u, vừa giảm nguy cơ ác tính, mang lại tiên lượng tốt cho người bệnh.

Rare hepatic schwannoma: A case report and literature review

Abstract
Schwannoma is a benign, slow-growing tumor derived from schwann cells, commonly occurring in the extremities, head and neck, mediastinum, retroperitoneum, and pelvis, while hepatic schwannoma is exceedingly rare. We report the case of a 53-year-old female patient admitted to the hospital with epigastric pain. Computed tomography and preoperative biopsy suggested a right hepatic tumor suspicious for schwannoma. The patient underwent right hepatectomy combined with extrahepatic bile duct resection and Roux-en-Y hepaticojejunostomy. Histopathological examination revealed spindle-shaped cells with oval, wavy nuclei arranged in fascicles, and immunohistochemistry showed positivity for S-100, consistent with hepatic schwannoma. The postoperative course was uneventful, and the patient was discharged in good condition. Hepatic schwannoma is a rare entity with nonspecific clinical and radiological findings, making preoperative diagnosis challenging. Definitive diagnosis relies on histopathology and immunohistochemistry. Complete surgical resection remains the treatment of choice, both to achieve curative removal and to minimize the risk of malignant transformation, thereby ensuring a favorable prognosis.
